Job summary

Pair Team is hiring a Field Care Coordinator to deliver in-home and community-based care for members under a major health plan. You will perform onboarding, distribute care packages, and collect health readings, traveling within Orange County on a regular basis.

The role demands in-person visits 3–5 times per week, strong bilingual communication, and a commitment to holistic, patient-centered support. You will join a field-based team that values integrity and collaboration.

Qualifications

• Bilingual – English/Spanish is required.
• 1+ years of field (in person) case management or care coordination.
• Physical location in or near Orange County with familiarity of the local community and resources.

Responsibilities

• Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
• Meet members in person at their homes, in the community, or at partner organizations to onboard, deliver care packages, or collect health readings.
• Conduct in-person visits to assess needs and ensure adherence to care plans.
• Coordinate services, appointments, and referrals across care teams.
• Collaborate with Nurses and Behavioral Health Care Managers to support care continuity.
• Identify and address barriers to care such as transportation or health literacy.
• Develop and maintain individualized care plans with regular follow-ups.
• Maintain accurate documentation of all interactions and coordination activities.
• Advocate for member needs within healthcare and social support systems.
• Participate in multidisciplinary care meetings to review patient panels and plans.
