Job summary

Pair Team seeks a Field Care Coordinator to bring care to members in their homes and communities across Orange County. This full-time, field-based role involves in-person visits, new patient onboardings, and outreach to overcome barriers to care.

Expect 3–5 patient visits per week, with travel 4 days a week and a Monday–Friday schedule. You will work with nurses and care managers to develop individualized care plans, coordinate services, and ensure timely follow-up, documentation, and patient

Qualifications

• 1+ years of field (in person) case management or care coordination.
• Physical location in or near Orange County, CA.
• Valid driver’s license and current auto liability insurance.
• Bilingual – English/Spanish required.

Responsibilities

• Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
• Meet members in person at homes, in the community, or at partner organizations to onboard and deliver care packages.
• Conduct in-person visits to assess needs, provide ongoing support, and encourage adherence to care plans.
• Coordinate services, appointments, and referrals across primary care, behavioral health, and social support resources.
• Collaborate with Nurses and Behavioral Health Care Managers to ensure care continuity.
• Identify and address barriers to care such as transportation, medication access, or health literacy challenges.
• Develop and maintain individualized care plans and ensure follow-up on all interventions and goals.
• Maintain accurate documentation of all interactions and care coordination activities.
• Advocate for member needs within healthcare and social support systems.
• Participate in multidisciplinary care meetings to review patient panels and align on care plans.
