Job summary

Pair Team is hiring a Field Care Coordinator in Orange County, CA. You will deliver in-home and community-based care, onboard new patients, and perform field outreach with a schedule of Monday to Friday, 9am to 5:30pm. Expect 4 travel days per week and 1 work-from-home day, with a caseload of 55.

Spanish fluency is required. You’ll partner with nurses and behavioral health managers to coordinate care, manage barriers, and ensure timely documentation. Salary is $30/hr with comprehensive benefits.

Qualifications

• Bilingual – English/Spanish required.
• 1+ year of field (in person) case management or care coordination.
• Physical location in or near Orange County with familiarity of the local community and resources.

Responsibilities

• Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
• Meet members in person at their homes, in the community, or at partner organizations to complete onboarding, deliver care packages, or collect health readings.
• Conduct in-person visits to assess needs and provide ongoing support and adherence to care plans.
• Coordinate services, appointments, and referrals across primary care, behavioral health, and social support resources.
• Support care continuity by collaborating with Nurses and Behavioral Health Care Managers.
• Identify and address barriers to care such as transportation or medication access.
• Develop and maintain individualized care plans and ensure follow-up on interventions.
• Maintain documentation of all member interactions and coordination activities.
• Advocate for member needs within healthcare and social support systems.
• Participate in collaborative care meetings to review patient panels and plan alignment.
