Job summary

Pair Team is seeking a Bilingual Field Care Coordinator in Orange County, CA. The role involves delivering hands-on, relationship-based support through in-person visits and onboarding for high-need members, with regular field travel and a 4-day travel schedule per week.

You’ll interact with members in homes and community settings to drop off care packages, collect readings, and ensure adherence to care plans. The position requires Spanish fluency and a strong caregiver mindset.

Qualifications

• 1+ years of field in-person case management or care coordination.
• Must reside in or near Orange County, CA.
• Reliable transportation for field visits.
• Valid driver’s license and auto liability insurance.
• Strong cultural fluency and empathy in diverse populations.
• Ability to work collaboratively in a multidisciplinary team.
• Excellent organizational skills and attention to detail.
• Comfort with Google Suite, Slack, CRM databases, and basic Excel/Word.
• HIPAA-compliant workspace and reliable internet for documentation.

Responsibilities

• Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
• Meet members in person at homes, community sites, or partner organizations to onboard and deliver care packages.
• Conduct in-person visits to assess needs and encourage adherence to care plans.
• Coordinate services, appointments, and referrals across primary care, behavioral health, and social supports.
• Collaborate with Nurses and Behavioral Health Care Managers to support care continuity.
• Identify and address barriers to care such as transportation or health literacy.
• Develop individualized care plans and follow up on all interventions and goals.
• Maintain accurate documentation of member interactions and care coordination activities.
• Advocate for member needs within healthcare and social support systems.
• Participate in care meetings to review patient panels and align on plans.
