Job summary

Pair Team is expanding its field-based care management program in Orange County. The Field Care Coordinator will visit members at home and in the community, complete onboarding, deliver care packages, and collect health readings.

You’ll travel four days a week and meet members 3–5 times weekly. This is a full-time, field-based position with regular travel and gas reimbursement. The role requires bilingual English/Spanish and a compassionate, collaborative approach.

Qualifications

• Bilingual English/Spanish required or strongly preferred.
• 1+ years field (in person) case management experience.
• Residence near or in Orange County with knowledge of local resources.
• Reliable transportation for regular field visits; valid driver’s license and auto insurance.
• Familiarity with HIPAA and maintaining patient confidentiality.
• Proven ability to use common productivity and collaboration tools.

Responsibilities

• Serve as the primary contact for members with complex needs.
• Meet members in home, community, or partner sites for onboarding and care packages.
• Conduct in-person visits to assess needs and support care plan adherence.
• Coordinate services and referrals across primary care, behavioral health, and social resources.
• Collaborate with nurses and care managers to ensure care continuity.
• Identify and address barriers to care like transportation or health literacy.
• Develop individualized care plans and follow up on interventions.
• Document all member interactions and care activities accurately.
• Advocate for member needs within healthcare and social systems.
• Participate in care team meetings to review patient panels and plans.
