Job summary

Pair Team in Orange County is seeking a Field Care Coordinator to deliver in-home and community-based support for high-need patients. You will conduct onboarding visits, care package deliveries, and health readings, with four travel days weekly.

This full-time role requires English/Spanish bilingualism, reliable transportation, and strong organizational skills. You will collaborate with Nurses and Behavioral Health Care Managers, document care coordination activities, and help members overcome

Qualifications

• Bilingual in English and Spanish.
• 1+ years of field in-person case management.
• Physical location in or near Orange County, CA.
• Reliable transportation for field visits.
• Valid driver's license and auto liability insurance.
• Strong cultural fluency and ability to build trust with diverse populations.
• Empathetic and patient, with resilience in challenging situations.
• Ability to work collaboratively in a multidisciplinary care team.
• Excellent organizational skills and attention to detail.
• Proficiency with Google Suite, Slack, CRM databases, and basic Excel/Word/Email/Video conferencing.

Responsibilities

• Serve as the primary point of contact for members with complex medical, behavioral, and social needs.
• Meet members in person at their homes, in the community, or at partner organizations to complete onboardings, deliver care packages, or collect health readings.
• Conduct in-person visits to assess needs, provide ongoing support, and encourage adherence to care plans.
• Coordinate services, appointments, and referrals across primary care, behavioral health, and social support resources.
• Support care continuity by collaborating with Nurses and Behavioral Health Care Managers.
• Identify and address barriers to care such as transportation, medication access, or health literacy challenges.
• Develop and maintain individualized care plans and ensure follow-up on all interventions and goals.
• Maintain accurate documentation of all member interactions and care coordination activities.
• Advocate for member needs within healthcare and social support systems.
• Participate in collaborative care meetings to review patient panels and align on care plans.
