Qualifications

• 1+ years of field (in person) case management or care coordination.

• Reliable transportation for regular field visits in Orange County.

Responsibilities

• Serve as primary contact for members with complex medical, behavioral, and social needs.
• Meet members in person at homes, community sites, or partner offices to onboard and deliver care items.
• Conduct in-person visits to assess needs and support adherence to care plans.
• Coordinate services, appointments, and referrals across care teams.
• Maintain timely documentation of member interactions and care coordination activities.
