Job summary

Pair Team seeks a Field Care Coordinator to bring care and support directly to patients in the community across Alameda County. You will conduct in-home visits, deliver care packages, onboard new patients, and address barriers to access, working five days per week in the field.

You will manage a busy schedule, perform ~30 visits per week, and collaborate with Nurses and Behavioral Health Care Managers to ensure comprehensive care. Bilingual English/Spanish and local knowledge are essential.

Qualifications

• Bilingual English/Spanish required or preferred.
• 1+ years of field-based (in-person) case management or care coordination.
• Residence in Alameda County or nearby with local community familiarity.
• Reliable transportation and a valid driver's license.
• Strong empathy, cultural fluency and ability to build trust with diverse populations.
• Comfort with Google Suite, Slack, CRM databases, and basic Excel/Word usage.

Responsibilities

• Manage a high-volume field-based caseload with ~30 in-person visits per week.
• Travel across Alameda County to meet patients where they are.
• Support community events and coordinate care for complex needs.
• Serve as primary contact for members with medical, behavioral, and social needs.
• Conduct home visits, deliver care packages, and collect health readings.
• Document all interactions and coordinate care plans with the team.
