Job summary

Qualifications

• Bachelor’s Degree in the Human Service field with 1 year of experience or training in social services, or 5 years of relevant experience.
• Experience with children and families with significant behavioral health needs preferred.
• Home visiting experience preferred.

Responsibilities

• Provide outreach and engagement to clients upon referral by phone intake and eligibility screening.
• Assess family needs and develop a care plan using Ohio scales during home visits.
• Provide weekly direct service hours to support parenting, safety planning, and life skills.
• Coordinate Child Specific Team meetings and follow through on recommendations.
• Maintain community relations and participate in external meetings and trainings.
• Maintain up-to-date case records including weekly reports and service agreements.
