Job summary

Qualifications

• Bachelor's in Human Services with 1 year (or training) in social services.
• OR 5 years of relevant Human Services experience.
• Experience with children and families with significant behavioral health needs.
• Home visiting experience preferred.

Responsibilities

• Provide outreach and engagement to referred clients and assess eligibility.
• Assess families and develop care plans using home visits and Ohio scales.
• Deliver weekly direct services: support, advocacy, referrals, and parenting guidance.
• Coordinate Child Specific Team meetings and implement recommended actions.
• Build community relations through education, advocacy, and external meetings.
• Participate in case reviews, supervision, trainings, and maintain case records.
