Job summary

UCFS Healthcare is seeking a Care Coordinator to support families with high-risk children, addressing behavioral or emotional issues, parenting challenges, and life stressors through bio-psycho-social assessments and service plans. The role involves travel within UCFS’s service area, direct client contact, and collaboration with the care team.

Qualifications

• Bachelor's Degree in Human Services or 5 years of relevant experience
• Experience with families and high-risk children; home visiting preferred
• Ability to travel 65–70% within UCFS Healthcare’s service area
• Experience in coordinating care and assessments is a plus

Responsibilities

• Provide outreach and engagement to clients upon referral and conduct initial intake to determine eligibility
• Assess family needs and develop a care plan using home visits and Ohio scales
• Deliver weekly direct services, advocacy, referrals, and support for parenting and life skills
• Coordinate Child Specific Team meetings and implement recommendations
• Maintain up-to-date case records and participate in supervision and trainings
