La Casa de Salud in the Bronx is seeking a Care Coordinator to bridge providers and patients, schedule internal and external services, and ensure follow-up with patients and tests. You will assist with data entry in the EMR and outreach to improve care engagement.

The role emphasizes bilingual communication (English/Spanish), collaboration with the care team, and culturally competent education for chronic disease prevention. This is a full-time, on-site position at LCDS sites.

Formación

• Two years' experience in Care Coordination.
• Bilingual English and Spanish preferred.
• Experience with EMR data entry and outreach.

Responsabilidades

• Facilitates interdisciplinary communication and planning.
• Schedules appointments for internal and external services.
• Conducts follow-ups to ensure appointments are kept and reports are available.
• Enter data in the EMR system.
• Conduct outreach for health measurements and engage with patients.
• Create effective linkages between vulnerable populations and the health care system.
• Provide culturally appropriate health education on chronic disease prevention, activity, and nutrition.
