Job summary

Clifford Beers Community Care Center is seeking a Care Coordinator to deliver bilingual, family-centered services to diverse families, in person, in-office and in the community settings.

You will work with an interdisciplinary team to engage families, develop plans of care, link services and resources, and promote engagement and resilience, ensuring information flows in the family’s preferred language (Spanish).

Qualifications

• Bachelor’s level degree in a behavioral health field preferred.
• Or 5 years experience.
• Must demonstrate family engagement and bilingual communication skills.
• Technical: knowledge of basic group skills and ability to delegate tasks; fully bilingual in Spanish and Latine cultures.

Responsibilities

• Establishing rapport with family members, schools and providers.
• Collaborate and participate in an interdisciplinary team related to client care.
• Engage families in the community and conduct home visits.
• Linking services and resources; ensure information flow in family language.
