Qualifications

Responsibilities

• Deliver weekly direct services, advocacy, referrals, and support for parenting and life skills
• Coordinate Child Specific Team meetings and implement recommendations
• Maintain up-to-date case records and participate in supervision and trainings

We are currently seeking a Care Coordinator who will be responsible for providing support services to families with high-risk children, regarding behavioral or emotional problems, parenting issues, and life stressors by developing and implementing bio-psycho-social assessments and services plans by performing the following duties:

UCFS Hours of Operation: Open 8:00am - 7:00pm; Hours Vary By Day & Location

(We are also open on the 2nd and 4th Saturday of each month)

Expected Schedule: Full-Time, 40 hours/week; Flexible

Outline of Duties and Responsibilities:

• Provide outreach and engagement to client upon referral to the program by conducting an initial phone intake process with the family, collecting basic information and presenting information to team to determine eligibility
• Complete an assessment of the family including both strengths and areas of need through home visits and Ohio scales to develop a care plan
• Provide weekly face to face direct service hours to client including support, advocacy, referrals and assistance with the development of positive parenting, safety planning, basic life skills and parenting a child with special needs
• Develop a list of interested parties identified by the family to participate in Child Specific Team meetings-run by the care coordinator- multiple times throughout involvement with the family and follow through with recommendations made at the Child Specific Team meetings
• Maintain and build community relations, through education and advocacy and participating in external meetings such as the System of Care meeting
• Participate in case reviews and supervision and attend all regularly scheduled team meetings and trainings
• Maintain up-to-date case records on each client family, including weekly contact reports, service agreements, evaluations and termination summaries

Requirements

Minimum Education and Experience

• Bachelors Degree in the Human Service field (such as Child Psychology, Social Work, Human Services) with a minimum of one years’ experience or training in social services
• OR 5 years of relevant Human Services experience
• Experience working with children and families with significant behavioral health need
• Home visiting experience highly preferred

Minimum Certificates, Licenses, Registrations

• A valid driver's license and state mandated levels of auto liability insurance
• Eligibility to obtain a CPR certification within 30 days of hire
• Eligibility to earn a Wraparound certification as soon as possible, considering when statewide trainings are offered

Minimum Skills and Abilities

• Ability for 65 – 70% travel within UCFS Healthcare’s service area
• Proficiency with Microsoft Suite, especially in Outlook
• Ability to provide a strength based, client-centered approach to the delivery of service
• Ability to work in a fast-paced environment and handle multiple tasks simultaneously
• Strong communication and teamwork skills
• Robust interpersonal expertise over digital, in-person, and verbal correspondence
