JEWISH FAMILY AND CHILDRENS SERVICE INC is hiring an Adult Case Manager in Mesa, AZ. The role provides ongoing case management to members designated as Seriously Mentally Ill, coordinating behavioral and physical health services and supporting transitions to adulthood.

Responsibilities include community outreach, transportation coordination, and ensuring access to primary care while adhering to agency procedures and patient rights.

Qualifications

• High school diploma required; bachelor's degree in a human services field preferred.
• Knowledge of mental health disorders such as schizophrenia, bipolar disorder, depression, PTSD, and substance use.
• Ability to obtain a Level 1 Fingerprint Clearance Card without restrictions.
• Arizona Driver's License and valid auto insurance; reliable transportation.

Responsibilities

• Provide ongoing case management to Seriously Mentally Ill (SMI) members in the community.
• Coordinate behavioral health services and care with physical health providers.
• Maintain communication with Clinical Team participants and document interventions.
• Navigate transition-to-adulthood referrals and patient rights processes.
• Collaborate with staff to ensure access to primary care and follow-through on referrals.
