Universal Community Health Center in Los Angeles is seeking a Patient Navigator to support patients in accessing community resources, social services, and medical systems. The role acts as a liaison to facilitate access to services through assessments, care coordination, and case management activities, including RPM program oversight.

Qualifications

• High school diploma required; bachelor’s degree preferred.
• 1 year of experience in community-level health education or related fields.
• Valid driver’s license and insurance; travel to patient homes and escort patients to medical appointments.
• Two years of experience with vulnerable populations, homeless individuals/families, and those with mental health disorders including substance use disorder.

• Collaborate with health education participants onsite and in the community.
