La Clinica de Familia in Las Cruces, NM is seeking a Patient Navigator–Community Health Worker to support clients as they navigate behavioral health, physical health, and social services. You will work with a multidisciplinary CCBHC team to improve health outcomes, reduce barriers to care, and promote recovery.

Qualifications

• High school diploma or equivalent.
• 3 years of direct patient care experience.
• Community Health Worker Certification or able to obtain within first 3–6 months of employment.
• Medical Assistant Certification, preferred.
• Experience working with diverse populations, including individuals with behavioral health or substance use needs, preferred.

Responsibilities

• Support clients as they navigate behavioral health, physical health and social service systems.
• Engage communities and coordinate care with a multidisciplinary CCBHC team.
• Advocate for culturally responsive and person-centered services to address social determinants of health.
