Universal Community Health Center (UCHC) is seeking a Patient Navigator to support patients by guiding access to community resources, social services, and medical systems. The role serves as a liaison between UCHC and the community to facilitate access through assessments, care coordination, and case management activities.

Responsibilities include assessments, care coordination, collaboration with the Quality Assurance Specialist, and oversight of the Remote Patient Monitoring (RPM) program,

Qualificações

• High school diploma required.
• Bachelor’s degree in related fields preferred.
• 1 year of community-level health education or related fields experience.
• Valid driver’s license and insurance coverage for travel to patient homes and appointments.
• 2 years of experience working with vulnerable populations, homeless individuals/families, and those with mental health or substance use disorders.

Responsabilidades

• Provide navigation across community resources, social services, and medical systems.
• Coordinate assessments, care coordination, and case management support.
• Collaborate with QA Specialist and oversee the Remote Patient Monitoring (RPM) program.
• Visit patients as needed and assist with forms to access services.
