Job summary

The Wesley Health Centers - JWCH Institute, Inc. in Los Angeles is seeking a Patient Navigator to warmly welcome clients, assist with forms, and guide patients through our administrative systems and services.

This role emphasizes a positive, patient-centered experience in a busy clinic environment. You will coordinate with the healthcare team, relay updates, educate clients about benefits, and refer eligible individuals to Enrollment Specialists, while occasionally traveling between site

Qualifications

• Minimum 1 year of front desk or receptionist experience.
• Ability to engage with clients and staff from diverse backgrounds.
• Bilingual English and Spanish preferred.
• Valid CA Driver's License with clean driving record.
• Willingness to travel between sites and work some evenings/weekends.
• COVID-19 vaccination recommended for JWCH staff.

Responsibilities

• Welcome clients with a friendly, professional demeanor.
• Identify informational needs through conversations with clients.
• Assist diverse clients in understanding service access and limitations.
• Coordinate communication between clients and healthcare team.
• Serve as liaison between clients and medical staff.
• Disseminate updates from front/back office staff promptly.
• Help clients complete forms and streamline intake processes.
• Provide information on on-site services and health programs.
• Refer eligible clients to Enrollment Specialists for benefits screening.
• Represent the clinic in healthcare service delivery matters with Clinic Administrator.
• Collaborate with outreach to greet new clients and assist intake.
• Participate in community outreach and maintain program logs.
• Assist with transportation of patients when needed.
• Attend staff meetings and follow safety/compliance trainings.
• Identify hazards and follow infection prevention procedures.
