Job summary

Community Health Partnership of Illinois (CHP) is seeking a compassionate, organized Patient Health Navigator to join our team in Aurora, Illinois. In this role, you will assist patients in navigating healthcare services, accessing community resources, and overcoming barriers to care.

You will collaborate with clinical staff and community organizations, maintain thorough documentation, protect patient confidentiality, and support referrals and follow-ups.

Qualifications

• High school diploma or equivalent; associate/bachelor in related field preferred.
• Experience in healthcare, patient navigation, or social services preferred.
• Strong communication, interpersonal, and organizational skills.
• Ability to work with patients from diverse backgrounds.
• Ability to maintain confidentiality and handle sensitive information.
• Basic computer and documentation skills.
• Bilingual/Spanish preferred.

Responsibilities

• Help patients navigate healthcare services and access resources.
• Identify barriers and connect to community resources.
• Support appointment scheduling, referrals, follow-up care, and coordination.
• Provide education about healthcare programs and services.
• Coordinate with clinical staff and other service providers.
• Maintain accurate documentation of patient interactions.
• Protect patient confidentiality and HIPAA compliance.
