Job summary

Family Health Care Centers of Greater Los Angeles is seeking a Patient Navigator to support care coordination and quality improvement initiatives in the greater Los Angeles area. You will work closely with FHCCGLA teams to identify patient needs within Q.I./Q.A.

goals and ensure access to necessary services. The role requires strong analytical and communication skills, Spanish proficiency, and a commitment to patient-centered care.

Qualifications

• High school diploma or GED required.
• FQHC experience is highly preferred.
• Excellent writing and verbal communication skills required.
• Knowledge of administrative principles and procedures.
• Ability to speak Spanish is required.
• Experience with diverse populations preferred.

Responsibilities

• Greet clients and provide health education.
• Pull reports to identify preventive care needs and follow up.
• Coordinate patient scheduling and flow to address bottlenecks.
• Assist clients with forms and intake processes.
• Maintain HIPAA compliance and confidentiality of records.
• Serve as liaison between patients and medical staff.
• Attend required meetings and trainings.
• Deliver high quality customer service to FHCCGLA patients.
