Job summary

Family Health Care Centers of Greater Los Angeles, Inc. is seeking a Patient Navigator to work closely with the Chief Medical Officer and healthcare teams to improve coordination of patient care. The role supports FHCCGLA patients by identifying needs aligned with Quality Improvement and HEDIS goals.

The Patient Navigator will manage scheduling, assist diverse clients, and ensure effective communication between patients and providers. Evening and weekend work may be required.

Qualifications

• High school diploma or GED required.
• Spanish language ability required.
• Excellent written and verbal communication skills expected.
• Strong administrative and clerical skills required.
• Experience serving diverse populations preferred.

Responsibilities

• Greet clients and visitors; manage front-desk interactions.
• Provide health education and instructions to patients as needed.
• Pull and interpret patient data from EHR systems to identify needed preventive care.
• Contact patients to follow up per QI/HEDIS goals.
• Assist in navigating patients through the health system and services.
