Malama Health in Louisiana seeks a Community Doula to serve as a Maternity Care Navigator, bridging providers, community organizations and patients to ensure coordinated, equitable care across the region.

You will conduct outreach at clinics and community sites, support appointments, assist with social services, and travel regularly to partner sites. English fluency is required and Spanish is a plus; compensation is $18–$23 per hour with flexible hours and room to grow.

Qualifications

• 1-2 years of experience as Community Health Worker, Community Doula, Case/Care Manager, or Social Worker.
• Must be a certified birth doula prior to employment (training, attendance at 3 births, CPR).
• Fluent in English; bilingual in Spanish preferred.
• Located in Louisiana; willing to travel regularly across counties.
• Experience with Medicaid or community-based maternal health programs.
• Strong communication, outreach, and relationship-building skills.
• Proficient with Google Workspace, Notion, Slack, Zoom.

Responsabilités

• Deliver Care Management to Meet Patient Needs by outreach, check-ins, and assessments via phone, SMS, video, or in-person.
• Coordinate with health plans and care teams to drive services to completion.
• Educate and empower patients using motivational interviewing and trauma-informed care.
• Document and track care coordination activities in portals; manage caseload.
• Engage with community-based organizations; staff events and outreach.
• Travel regularly to partner sites for outreach and visits.
