Malama Health in Michigan is seeking a Community Doula to act as a Maternity Care Navigator, using our tools to support pregnant and postpartum mothers. The role focuses on bridging providers, community organizations and patients to ensure coordinated, equity-driven care.

You will conduct outreach at clinics and community sites, coordinate services, document care in portals, and travel across counties. Requirements include 1–2 years in related roles and birth-doula certification; English fluency

Qualifikationen

• 1-2 years experience as a Community Health Worker, Community Doula, Case/Care Manager, or Social Worker
• Must be a certified birth doula prior to employment (training, attendance at 3 births, CPR, etc.)
• Fluent in English; bilingual in Spanish preferred
• Comfortable using Google Workspace, Notion, Slack, Zoom

Aufgaben

• Deliver Care Management to Meet Patient Needs via outreach, check-ins, and assessments
• Coordinate Care Services and Referrals to Completion with health plans and providers
• Communicate to Empower Patient Learning using motivational interviewing and trauma-informed care
• Utilize care management portals to document and track services and meet caseload targets
• Conduct in-person outreach at clinics and community sites across assigned counties and organize events
