Job summary

Malama Health, Inc. is seeking a Community Doula to serve as a Maternity Care Navigator, delivering compassionate outreach and care coordination for pregnant and postpartum families in Michigan.

You will build relationships with community-based organizations, clinics, and families, run in-person and virtual outreach, and help connect members to services through our technology platforms, while traveling across counties.

Qualifications

• 1-2 years of experience as Community Health Worker, Community Doula, Case/Care Manager, or Social Worker.
• Must be a certified birth doula prior to employment (training, attendance at 3 births, CPR, etc.)
• Reliable and responsive to text and calls; comfortable using Google Workspace, Notion, Slack, Zoom.
• Fluent in English and bilingual in Spanish (written and verbal).
• Located in Michigan and able to travel regularly across assigned counties.

Responsibilities

• Deliver Care Management to meet patient needs via outreach, check-ins, and assessments.
• Coordinate care services and referrals with health plans and care teams to drive services to completion.
• Communicate to empower patient learning using motivational interviewing and trauma-informed care.
• Utilize care management portals to document, track, and improve services; manage caseload targets.
• Conduct in-person outreach at clinics and community sites, and staff events to highlight Malama's services.
