Job summary

Malama Health in Louisiana is seeking a Community Doula to serve as a Maternity Care Navigator, performing outreach, care coordination and patient support using our tech tools. You will work with pregnant and postpartum members, bridging providers, community-based organizations, and families to ensure timely, equitable care.

The role requires 1–2 years in community health or related fields, certification as a birth doula, fluency in English and Spanish, and comfortable travel across counties.

Qualifications

• 1–2 years of experience as Community Health Worker, Community Doula, Case/Care Manager, or Social Worker.
• Certified birth doula prior to employment (training, attendance at 3 births, CPR).
• Reliable and responsive to text and calls; comfortable with Google Workspace, Notion, Slack, Zoom.
• People person and team player; Be the Bridge values; empathetic and constructive feedback.
• Mission-driven with passion for health, technology, and birth equity.
• Deep ties to the communities served; ability to travel regularly across counties.
• Experience with Medicaid or community-based maternal health programs.
• Fluent in English and bilingual in Spanish; Louisiana-based.

Responsibilities

• Deliver Care Management to meet patient needs via outreach, assessments, and coordination.
• Coordinate care services and referrals to completion with health plans and providers.
• Communicate to empower patient learning using motivational interviewing and trauma-informed care.
• Document and track services using care management portals; manage caseloads.
• Conduct community outreach at clinics and events; organize staff for events.
