Job summary

Malama Health is hiring a Community Doula in Louisiana to serve as a Maternity Care Navigator, delivering personalized care coordination and outreach in local communities. You’ll work with pregnant and postpartum members, clinicians, and community partners to close equity gaps and improve outcomes.

Responsibilities include direct outreach, care coordination across plans and providers, and documenting activity using care-management tools. Flexibility and travel across counties are expected.

Qualifications

• 1–2 years of experience as a Community Health Worker, Community Doula, Case/Care Manager, or Social Worker.
• Must be a certified birth doula prior to employment (training, attendance at 3 births, CPR, etc.).

Responsibilities

• Deliver care management by conducting outreach, assessments, and scheduling appointments via multiple channels.
• Coordinate care services with health plans and care teams to drive services to completion.
• Empower patients through motivational interviewing, trauma-informed care, and harm reduction.
• Document and track services using care management portals and manage caseloads.
• Engage with communities through in-person outreach, events, and partnerships.
