Malama Health is seeking a Community Doula in Michigan to serve members as Maternity Care Navigators, bridging providers, community organizations and patients. You will outreach, coordinate care, and document progress using our tools, helping close equity gaps in maternal health.

Qualifications

• 1-2 years of experience in Community Health roles or care management.
• Must be a certified birth doula prior to employment and CPR trained.
• Fluent in English and bilingual in Spanish.

Responsibilities

• Deliver Care Management to meet patient needs via outreach, assessments, and coordination.
• Coordinate referrals with health plans and care teams to completion.
• Use technology to document, track, and improve services.
• Engage with community events and partner sites for outreach.
