National Youth Advocate Program seeks a dedicated Case Manager to connect youth and families with essential services. The role emphasizes timely assessments, coordinated care, and collaboration with sponsors, attorneys, and multidisciplinary teams to support reunification and safety.

The successful candidate will navigate complex eligibility requirements, document thoroughly, and maintain strong relationships with foster families and partners across the U.S.

Qualifications

• Bachelor’s degree required in Social Work, Psychology, Human Services, Counseling or other social service field.
• 1 year experience preferred working with children/adolescents in youth services.
• Excellent case management, verbal and written communication skills.

Responsabilités

• Complete assessments and service plans within required timeframes.
• Conduct intake interviews with youth and sponsors/family.
• Coordinate with sponsors, legal entities and multi-disciplinary teams.
• Maintain case files compliant with ORR policy and NYAP CQI.
• Submit reunification packets and referral documents for timely release of youth.
• Provide weekly updates to youth and families via phone or in person.
