Job summary

National Youth Advocate Program seeks a dedicated Case Manager in Gurnee, IL to connect youth and families with essential resources. This nonprofit role focuses on timely assessments, sponsor vetting, and coordinating care with a multidisciplinary team in a dynamic environment.

Qualifications

• Bachelor’s degree in social work or related field.
• 1 year experience with children or youth services preferred.
• Strong communication, organizational and multitasking abilities.
• Willingness to obtain CPI and First Aid certifications.

Responsibilities

• Perform case management in line with NYAP mission and policies.
• Ensure ORR timelines for assessments are met.
• Conduct on-site intake interviews and gather sponsorship info.
• Interview sponsors/family to assess care capabilities for minors.
• Coordinate with Case Coordinators and partners on staffing and care plans.
• Lead pre-staffing with multidisciplinary team and document proceedings.
• Attend staffings with ORR stakeholders as needed.
• Coordinate with local attorneys for Know Your Rights presentations.
