Job summary

National Youth Advocate Program (NYAP) is seeking a dedicated Case Manager to connect youth and families with essential support and resources. The role requires coordinating assessments, service plans, and reunification efforts while navigating a multi-disciplinary team.

You will conduct intake interviews, work with sponsors, and ensure timely documentation. The position demands flexible hours, strong advocacy for youth, and collaboration across departments.

Qualifications

• Bachelor’s degree required in Social Work, Psychology, Human Services, Counseling or related field.
• One year experience preferred working with children/adolescents or youth services (volunteer/internship included).
• Ability to work flexible hours including weekends/evenings.
• Proficient in word processing, spreadsheets and databases.

Responsibilities

• Complete and submit timely assessments and service plans.
• Maintain comprehensive case files compliant with ORR policy and NYAP CQI requirements.
• Conduct intake interviews to gather sponsor information and determine youth age.
• Coordinate with sponsors, family members and stakeholders.
• Prepare cases for staffing and lead multi-disciplinary teams.
• Attend staffings with Case Coordinators and Federal Field Specialists as needed.
• Coordinate Know Your Rights presentations and legal screenings with local pro bono attorneys.
• Document safety plans, discharge planning and reunification processes.
• Provide weekly updates to youth and family members.
• Manage UC electronic/physical files and portal uploads.
