Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy for patients with behavioral health needs within primary care clinics. You will coordinate with care teams and a psychiatric consultant, assess patient needs, and refer to internal or external resources for evidence-based treatment.

The role may include remote work days based on program metrics.

Qualifications

• Master’s Degree in mental health field from an accredited school.
• Highest clinical license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiarity with brief, structured interventions (e.g., Motivational Interviewing, Behavioral Activation, Cognitive Behavioral).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to identify appropriate patients for referral to the Behavioral Health Program.
• Screen and assess patients for common mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultant or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultant and telehealth sessions; update care plans.
• Maintain accurate documentation of patient encounters in the EMR.
• Participate in regional efforts to advance the program and gather feedback.
