Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term evidence-based psychotherapy to patients with behavioral health needs. You will collaborate with Care Teams across primary clinics, assess patients, and coordinate referrals to our psychiatric consultant or external resources.

Qualifications

• Experience screening for common mental health and/or substance use disorders.
• Experience with assessment and treatment planning for these disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, CBT).
• Proficiency in performing care coordination with psychiatric consultants.

Responsibilities

• Collaborate with primary care teams to identify patients for referral to behavioral health programs.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including telehealth handoffs and care plan collaboration.
• Document patient encounters accurately in the EMR and contribute to program metrics.
