Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management services and short-term psychotherapy to patients with behavioral health needs. You will work with Care Teams across Oak Street Health clinics to coordinate care and referrals.

The role supports assessment, treatment planning, and collaboration with a psychiatric consultant, with potential remote work days based on performance and local leadership decisions.

Qualifications

• Master's degree in mental health counseling/healing arts from an accredited program.
• Highest clinical license or local equivalent.
• Experience screening and treating common mental health or substance use disorders.

Responsibilities

• Collaborate with primary care teams to refer patients to the behavioral health program.
• Screen and assess patients for mental health and substance use disorders.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Document patient encounters accurately in the EMR.
• Participate in regional efforts to advance the behavioral health program.
