CVS Health is seeking a clinical professional to apply advanced judgement in coordinating physical and behavioral health care. You will direct provider collaboration, plan care, and coordinate psychosocial services while maximizing outcomes and cost efficiency.

The role requires active licensure (LCSW/LPC/LMFT), 125-mile travel capability, a 40-hour work week, and a 2-day-per-week evening schedule. Comprehensive benefits and a base salary are offered.

Qualifications

• Current/Active Clinical Licensure (LCSW, LPC, LMFT)
• Case Management experience
• Strong customer service orientation and problem solving
• Excellent Communication/Telephonic skills
• Excellent motivational interviewing skills/ability to build rapport and trust telephonically
• Excellence in documentation and respecting compliance/regulatory standards
• Highly organized, self-driven without direct supervision
• Manage time effectively
• Achieve performance metrics
• Ability to travel within 125 miles round trip for 1 day in the field
• Variable work scheduled with ability to work 2 days weekly until 9pm local time
