Ideal candidates will have a master’s degree with licensure (LCSW, LPC, LMFT), proven case management experience, strong communication on telephonic platforms, and the ability to travel within 125 miles for 1 day.

• Current/Active Clinical Licensure (LCSW, LPC, LMFT)

Responsibilities

• Implements strategy to assess and plan care, direct provider collaboration, and coordinate psychosocial wraparound services.
• Instructs the application of available resources to produce optimal, cost-effective outcomes.
