Job summary

Adult & Child Health, in partnership with Brightli, seeks a Care Coordinator for Psychiatry in Indianapolis. You will serve as the central support for clients receiving outpatient behavioral health services, coordinating resources, and advocating to reduce barriers to care.

Responsibilities include collaboration with treatment teams, documentation, discharge planning, and crisis support. A Bachelor’s in a human services field is preferred, with strong communication and organization skills

Qualifications

• Bachelor’s degree in a human services field or related discipline.
• Experience working with behavioral health, substance use treatment, or community-based services preferred.
• Background screening including criminal, driving, abuse/neglect checks required.

Responsibilities

• Coordinate outpatient behavioral health and substance use services.
• Assist clients and families in accessing community resources and supports.
• Advocate for clients and help remove barriers to care.
• Maintain regular communication with referral sources and treatment teams.
• Ensure accurate and timely documentation of services and treatment plans.
