Job summary

Brightli is seeking a qualified professional to coordinate services for patients receiving mental health and substance use treatment. The role includes developing individualized treatment plans, communicating with referral sources, and ensuring proper documentation and crisis intervention as needed.

Responsibilities emphasize advocacy for families, linkage to community services, and collaboration with clinical teams to support patient progress and transition planning.

Qualifications

• Knowledge of case management and rehabilitation approaches for mental health and/or substance use disorders.
• Understanding of community resources and how to access them for clients.
• Familiarity with legal and court procedures related to treatment when applicable.

Responsibilities

• Coordinate outpatient mental health and/or substance use disorder services as needed.
• Maintain and update individualized treatment plans for clients.
• Document all services and communication with involved providers per standards.
• Provide crisis intervention as necessary and participate in staffing to assure continuity of care.
• Advocate for families and link them to community resources and transportation when needed.
