Responsibilities

• Care Coordination & Client Support: review assessments and identify needs across healthcare, housing, employment, education, social support, and legal systems; coordinate outpatient behavioral health and substance use services; advocate to remove barriers to care.
• Collaboration & Communication: maintain regular communication with referral sources, primary care providers, and treatment teams; participate in team meetings and coordinate referrals.
• Treatment Planning & Documentation: support development of individualized treatment plans and ensure accurate documentation; assist with transition and discharge planning.
• Crisis & Client Support: provide crisis intervention and help coordinate transportation and referrals.
