Job summary

Brightli is seeking a Care Coordinator – Psychiatry in Indianapolis to help clients navigate outpatient behavioral health services. You will coordinate care, support families, and liaise with clinicians to remove barriers to treatment and promote recovery.

Successful candidates will enable strong client engagement, meticulous documentation, and proactive coordination across healthcare and social service systems to improve wellbeing and stability.

Qualifications

• Knowledge of case management and rehabilitation practices in behavioral health or substance use treatment.
• Understanding of community resources and support systems.
• Ability to collaborate with multidisciplinary teams.
• Strong communication, organization, and documentation skills.
• Ability to advocate effectively for client needs.
• Bachelor’s degree in a human services field is preferred.

Responsibilities

• Serve as a central point of support for clients receiving outpatient behavioral health services.
• Review initial assessments and identify client needs related to healthcare, housing, employment, education, social support, and legal systems.
• Coordinate outpatient behavioral health and substance use services.
• Assist clients and families in accessing community resources and supportive services.
• Advocate for clients and help remove barriers to care.
• Maintain regular communication with referral sources, primary care providers, and treatment teams.
• Participate in treatment team meetings and clinical staffings.
• Coordinate referrals to outside providers when additional services are needed.
• Support the development and ongoing updates of individualized treatment plans.
