Eskenazi Health's Sandra Eskenazi Mental Health Center in Indianapolis is seeking a Behavioral Health Care Coordinator to support diverse populations across programs. You will help clients build skills, access services, and develop treatment plans with the treatment team in office, home, and community settings.

The role emphasizes recovery-oriented care, documentation, and collaboration with families and community supports.

Qualifications

• Bachelor’s degree required.
• Licensed Bachelor’s Social Worker license in Indiana if required by program.
• Valid Indiana driver’s license and driving record meeting policy.

Responsibilities

• Embraces the Recovery Model and uses motivational interviewing techniques.
• Conducts client assessments and revises treatment plans with the treatment team.
• Provides skills training and case management to support recovery and wellbeing.
• Delivers services in office, home, and community settings as needed.
• Makes timely documentation and billing entries per guidelines.
• Maintains safety and intervenes in crisis as required.
• Coordinates with families and community supports to meet goals.

Education

Bachelor’s degree

Licensed Bachelor’s Social Worker (LBSW) licensed in Indiana
