Qualifications

• Bachelor’s degree in a human services field is preferred.
• Experience in behavioral health or substance use treatment is preferred.
• Strong communication, organization, and documentation skills are required or strongly preferred.

Responsibilities

• Coordinate outpatient behavioral health and substance use services.
• Assist clients and families in accessing community resources and supportive services.
• Advocate for clients and help remove barriers to care.
• Maintain regular communication with referral sources, primary care providers and treatment teams.
• Participate in treatment team meetings and clinical staffings.
• Coordinate referrals to outside providers when additional services are needed.
• Support development and updates of individualized treatment plans and ensure timely documentation.
