Job summary

Adult & Child Health is seeking a Care Coordinator to manage coordination of services for patients receiving Medication Assisted Treatment in Indianapolis. You will review assessments, coordinate outpatient services, communicate with referral sources, arrange transportation, and develop individualized treatment plans while representing the agency professionally.

Requirements include a Bachelor’s degree in a human services field and knowledge of case management.

Qualifications

• Knowledge of case management and rehabilitation approaches in mental health/substance use.
• Strong written and verbal communication skills for clear reports and coordination.
• Ability to coordinate outpatient services and collaborate with referrals.
