Job summary

Adult & Child Health is seeking a Care Coordinator for Psychiatry in Indianapolis, IN. You will serve as a central point of contact to coordinate outpatient behavioral health and substance use services, review assessments, and connect clients with housing, employment, education, and community resources.

You will collaborate with clinicians and community partners, advocate for clients, and help remove barriers to care while maintaining thorough documentation and care plans.

Qualifications

• Knowledge of case management and rehabilitation in behavioral health or substance use treatment.
• Understanding of community resources and support systems.
• Ability to collaborate with multidisciplinary teams.
• Strong communication, organization, and documentation skills.

Responsibilities

• Care Coordination & Client Support: review assessments and identify needs across healthcare, housing, employment, education, social support and legal systems.
• Coordinate outpatient behavioral health and substance use services.
• Assist clients and families in accessing community resources and supportive services.
• Advocate for clients and help remove barriers to care.
• Collaboration & Communication: maintain regular communication with referral sources, primary care providers, and treatment teams; participate in team meetings.
• Coordinate referrals to outside providers when needed; represent the organization professionally.
• Treatment Planning & Documentation: support development and updates of treatment plans; ensure timely documentation; address chart deficiencies; assist with discharge planning.
• Crisis & Client Support: provide crisis intervention; coordinate transportation and scheduling for services; ensure referrals for medical needs.
