Job summary

Centerstone in Indianapolis, IN is seeking a Care Coordinator-Promoting Integration to coordinate services for patients undergoing Medication Assisted Treatment. The role emphasizes early assessment, discharge planning, and seamless coordination of mental health and substance use services.

Qualifications

• Knowledge of case management and rehabilitation methods related to mental health and/or substance use disorders.
• Understanding of community resources and referral processes.
• Awareness of legal requirements and procedures in treating mental illness and substance use disorders.

Responsibilities

• Review initial assessment and discharge planning and identify related issues.
• Coordinate outpatient mental health and/or substance use services as needed.
• Communicate with referral sources and primary care about progress and planning; document accordingly.
• Assist in scheduling treatment and arranging transportation when needed.
• Develop individualized treatment plans and participate in staffings to ensure continuity of care.
• Represent the agency professionally and advocate for families and clients.
