Lighthouse Behavioral Wellness Center is seeking a Care Coordinator to support individuals and groups in outpatient and community settings. You will advocate, link consumers to services after crisis or hospitalization, and help promote wellness across the Eight Dimensions of Wellness.

The role emphasizes coordinated care, culturally sensitive engagement, and collaboration with an integrated team to improve outcomes and stability for clients with mental health or substance use concerns.

Qualifications

• Bachelor or Master in Education, Psychology, Social Work, Sociology, Criminal Justice or similar behavioral health field.
• RN with behavioral health experience may qualify in Oklahoma.
• Any field with 6+ months behavioral health experience acceptable.
• ODMHSAS certifications for Behavioral Health Case Manager II and Wellness Coach preferred.

Responsabilités

• Provide case management, rehabilitation and wellness services in various settings.
• Deliver crisis management and stabilization as assigned.
• Coordinate care with internal and external providers.
• Develop care plans and monitor progress with consumer involvement.
• Refer to housing, employment, medical, nutrition, and other resources.
