The Community Health & Dental Care, Inc. is seeking a Behavioral Health LPN Coordinator to support care coordination within our behavioral health team.

You will collaborate with psychiatric providers, care teams, and community resources to ensure timely, integrated services and strong patient engagement. Formación

• Active unrestricted LPN license in Pennsylvania.
• Minimum of two years of nursing experience.
• Behavioral health, care coordination, or case management experience preferred.
• Experience with electronic health records.
• Bilingual in English and Spanish preferred.
• Strong organizational, communication, and interpersonal skills.

Responsabilidades

• Coordinate behavioral health services and patient care activities, including referrals, new patient appointments, follow-up visits, telepsychiatry services, and communication among providers and care team members.
• Assist patients in navigating behavioral health services by providing education, support, resource coordination, and follow-up to promote engagement and continuity of care.
• Respond to patient inquiries, gather pertinent clinical information, assist with medication refill processes within scope of practice, and communicate patient concerns to appropriate providers.
• Collaborate with providers, behavioral health staff, and community resources to support coordinated, patient-centered care.
• Room and prepare patients for scheduled appointments.
• Prepare and administer medications and injections as directed by providers.
• Maintains accurate and timely documentation in the electronic health record and assists with data collection, chart audits, reporting requirements, and regulatory compliance activities.
• Participates in quality improvement activities and supports departmental and organizational initiatives to improve patient outcomes and access to care.
