Job summary

Advantage Healthcare Services seeks a Community Health Liaison to support AHCS Enhanced Clinical and Support Service Programs, working with behavioral health patients in clinic, parole, and home-based settings to improve medication adherence and health outcomes. You will coordinate care with clinical and pharmacy teams and assist in identifying gaps to enhance treatment effectiveness.

Qualifications

• Experience in a community-based setting 1–2 years.
• Roles in Medical Assistant, CHW, Social Worker, Pharmacy Technician or equivalent.
• Successful completion of a Community Health Worker training program.
• Medical terminology or healthcare background.

Responsibilities

• Conduct Enhanced Clinical Services to AHCS patients.
• Offer MTM and other clinical services to optimize care.
• Conduct in-person patient visits for enhanced services programs at partner organizations.
• Coordinate care with patients' healthcare providers and care team.
• Optimize outcomes in patient adherence and prescription access.
• Attend meetings and work with other clinical team members.
• Complete required reporting in a timely manner.
• Act as liaison between patient and pharmacy to ensure adherence.
• Assist pharmacy teams with insurance coverage lapses and benefit enrollment.
• Collaborate with AHCS sales team on potential new referrals, leads and customers.
• Perform other tasks as required.
