Job summary

Lighthouse Behavioral Wellness Centers is seeking a Care Coordinator to provide case management, rehabilitation and wellness services, linking consumers to community resources after crises or hospitalization. You will work under supervision to support individuals and families across settings.

The role emphasizes collaboration with an integrated team, engagement in the community, and adherence to ODMHSAS standards for behavioral health care.

Qualifications

• Knowledge of social work/psychology and current social welfare resources.
• Understanding of Eight Dimensions of Wellness and personal wellness concepts.
• Ability to coordinate with multiple providers and monitor progress.

Responsibilities

• Provide case management, rehabilitation and wellness services in various settings.
• Offer crisis management and stabilization as assigned.
• Assess consumer needs, develop care plans and coordinate care with providers.
• Complete all agency documentation thoroughly and timely.
• Assist clients to adapt to their environment and desired goals.
