Job summary

Lighthouse Behavioral Wellness Centers is seeking a dedicated Care Coordinator to empower families navigating mental illness, trauma or addiction. Under supervision, you will organize the Child and Family Team, coordinate services and supports, and help families create strength‑based, culturally informed care plans.

The role requires travel to schools and communities, home visits when needed, and flexible hours to meet family needs. Strong collaboration and communication are essential.

Qualifications

• Education or license as required by ODMHSAS and related standards.
• Ability to provide documented proof of behavioral health experience.
• Willingness to obtain required certifications within three months.

Responsibilities

• Provide varied case management, rehabilitation and wellness services.
• Schedule and facilitate wraparound family team meetings.
• Enter data into the YIS system per ODMHSAS timelines.
• Offer crisis management and stabilization as assigned.
• Develop care plans and coordinate with internal and external providers.
